Regarding the Challenger's and other O-rings, if you fly them in weather that will freeze 'em like they did, you can pretty well count on the damn things failing if they have to restrain any kind of high pressure. However, I doubt that we subject the ones in our airplanes to duty approaching what's required in the joints of the shuttle's solid rocket casings. Long story short, if you've got leaking O-rings, they're probably physically damaged - most likely cause: installation.
