Rick,
Failure occurred before I even knew the washer existed.  In fact, I
rebuilt the cylinder replacing all seals expect the washer.  It leaked
just as before and thus the only possible leak path remaining was past
the shaft/piston interface.  It is only then that I discovered the
damaged sealing washer.
