Contributing factors were the poor layout of the instrument panel, the omission of a rotating mass (gyro) standby attitude indicator and the pilot's apparent failure to activate pitot heat.
Brent,
    where does the lack of gyron come in to play?  are you saying a CFS doesn't know which way is up if the pitot is blocked?
