I made this panel annunciator grouping from Spruce annunciator lamps (see
catalog), with 12 holes drilled in the aluminum.  The push-to-test feature
is the guts of the annunciator set (black button seen to the left of
grouping), requiring a circuit board, and also manages power and warning
inputs to the individual lamps.  I commissioned a 12 lamp push-to-test
circuit, and bought three.  I have two left.  I think I paid $110 each.
The Florida guy who made mine had commonly made them with 10 lamps, and
might do another custom with something different.  The labels on lamps were
made separately for ~$4 each.
