Tom, short answer...no. The required items are 1. Manufacturer (you) 2. Model and 3. Serial number (assigned by the manufacturer, you). Anything else is extra. I've got my info on a little 1"x2" plate. Address, date of birth, number of children, favorite color, etc. are all extra. You can certainly put it on if you want, but it's not required.
