On Steve Hall's IVP that recently crashed short on final here in CO, he had the cowling off a few hours before the shaft failed.  He reported that there were no engine leaks or vibration.  It seems as though these engines can fail without notice.  He shared with me that there was a mix up in ordering knowing shafts were bad.  The "bad" shafts were not removed and ended up going out on some of the engines.  You might want to keep after Continental.  I don't know any particulars of his pending case name wise with Continental.  This is just what he shared with me.
