The bolt is hollow with two holes in its stem. The external fitting is a circle fitting which has a passage around the bolt so the liquid can flow from it into the hollow stem of the bolt through both holes and into the block or whatever. Its advantages are that it is somewhat more compact than most fittings and can be put into tight spaces. Also it is somewhat more resistant to the effects of vibration. The down side is that it is more restrictive to flow than straight in fittings - and can be very hard to find as they are generally specially made for a specific application.
