There are infamous tales of parts going back to Beech and Cessna (I'm certain others too) after major failures, being requested for "Examination" and then disappearing forever.  Who can blame them?  It failed on an airplane--there's no upside to advertising that problem.    Some stories are incredible.  No wonder ntsb accompany all the evidence as it is tested by the manufacturers.
