 Scotty, just be sure to have all the forms filled out for your medical &
things will go smooth at your next phsyical, my AME just checked the forms for
correctness and issued my 3rd on the spot.  Had mine done in 2002.
 
 Tom, if a doc is needed in the  Bay Area, try Dr. David F. Chang in Los
Altos.  He's rated as one of the 10 best opthamologists in Bay Area and
specializes in difficult cases that other opthalmologists don't like to tackle
-- that was me.
