From what Vern as told me, if we never remove the 'nut', it would be very unlikely that this washer/o-ring will need replacing.  

So my question to you is: Did you remove the 'nut' as well PRIOR to the failure or was it the other way around (i.e. failure first, nut never remove before failure).
