I have mostly Honeywell NT series switches.  The TL series have gold plated contacts and should not be used for loads that may arch during switching. Not the most attractive or least expensive option. 

If I remember correctly Dennis dissembled his old switches and found evidence of arching or discoloration on the contacts.

I want to thank Steven Boese, Bobby Hughes and Mark Steitle for all their help in diagnosing this problem.
 
I never could make the switch fail during ground run testing!
