The Embry Riddle aeronautical students have to do a capstone project to graduate that includes a survey. I've seen many of these, many of them BS and poorly written/designed. This one is actually pretty decent, and asks the right questions, as well as leaves room for individualized responses. He's an EAA member and a builder, so I dont mind actually forwarding his request for surveys.
